Looks like a problem on the oxygen sensor.
Did you check your fuel trims (long term)? Problems on the oxygen sensors usually produce strange fuel trims (too high or too low) indicating that your engine is not being able to compensate the fuel injection based on O2S readings.
The test monitor also includes a mode that displays the realtime waveform for the O2S readings. It should have a sinusoidal-like shape, indicating the transitions from lean to rich and then from rich to lean.

The last time I had a P0455 on a Ford it involved a
hide-a-key gas cap.
